For those who completed the on-line evaluation, CPD credit confirmation emails were sent out September 2009. If you did not receive your confirmation email, please contact Inika Anderson, Manager CPD, as soon as possible.
If you did NOT complete the on-line evaluation, you must now track your credits manually. Simply download this form and check off the sessions you attended. Keep this form with your CPD records.
